"Can Roman Catholics receive communion in the Polish National Catholic Church? Also, what sacraments are different in the two churches?"

Online research indicates that this organization is not part of the Catholic Church. Traditionally such a situation is referred to as heresy or schism (depending on the nature of the break). For more information on heresy / schism, please see here.

Catholics should NOT receive sacraments from heretical / schismatic groups, as the sacraments are generally either invalid or illicit. For more information on what makes a sacrament valid, please see here. Especially note the question "Are There Sacraments Outside the Church?"

Also, please remember that Sacraments which require priestly powers can only be conferred by validly ordained priests. Furthermore it should be noted that, "the ecclesial communities which have not preserved the valid Episcopate and the genuine and integral substance of the Eucharistic mystery, are not Churches in the proper sense." Therefore, it is generally best to avoid the term 'church' when not speaking of the Catholic Church.
